Assistance with medications

If assessed as a requirement staff will assist you with your medications. In this instance your medications will be provided in a dosette package to allow for safe administration by our personal care staff. Your medications will be stored on-site in suitable, locked receptacles.  If you wish to self-medicate we will request from your medical provider an assessment that you are safe to do so and you must store your medications in a locked drawer in your room to prevent harm to others

Personal Care

Care staff will provide you with standby assistance and supervision with showering nail care and hair care. Service staff strictly adheres to a no lift policy. We will monitor and report any change to your health status, including pain, infection and wounds, to your health care provider. We will provide support in arranging appointments to see your health care providers. We cannot offer staff escorts or transport to attend appointments. If you have a diagnosed psychiatric illness, acquired brain injury, cognitive impairment/ dementia we will monitor your mood, cognition and behaviour and update your support plan to guide your care. We will also liaise with your health care providers. If referral to another service may assist to maintain your health and or wellbeing, this will be discussed with you and or your representative. Our staff cannot provide care management or coordination services for external health or disability providers you access and use, which includes conducting assessments on their behalf or organising staffing visits from them to you. 

Meal Planning and Dietary Support

The service will provide meals according to your religious practice and custom e.g. Halal, Kosher  The service will provide meals according to your diagnosed medical requirements i.e. diabetic, low salt The service will provide meals as per your listed sensitivities and allergies.. Managing your pain 

  • Breakfast is at 7:00 am. 
  • Lunch is at 12:00 pm. 
  • Afternoon tea is at 2:00 pm. 
  • Dinner is at 5:00 pm. 
  • Supper is at 7:00pm.

Brought In Food 

Whilst this is your home, due to food safety requirements staff cannot be responsible for food brought in by you or your family, visitors and friends. 

Medical Services

Medical Practitioner: You are encouraged to choose the medical practitioner of your choice. Ideally, your own General Practitioner will be able to continue to provide care however if this is not possible, we can provide a list of visiting doctors. we manages the provision of allied health professionals as required by the resident and the family. We can arrange regular on-site visits.

Support Planning

Here at Crystal Manor you are treated with dignity and respect and can maintain your identity. You can make informed choice about our care and services and live the life you choose.  We are committed to providing care and services that meet your needs and preferences for your health and well-being. To enable this to occur we work in partnership with you and your nominated representative to conduct an initial assessment and develop an individualised Support Plan that takes into consideration your needs, preferences, safety and your right to take risks. Within 48 hours of your admission to the service an interim support plan will be prepared for you that includes your immediate health and support needs and the services we will provide you to assist with those needs. Upon request a copy of the interim support plan will be provided to you, your nominated representative and your health provider. Within 28 days of your admission the service will, in consultation with you and if appropriate your nominated representative and health provider, prepare an ongoing support plan. The support plan will include your on-going health and personal support needs and the services we will provide to assist you with those needs.

ALLIED HEALTH PROFESSIONALS

We manages the provision of allied professionals as required by the resident and family. we can arrange regular on-site visits Assessment and rehabilitation may be available through referral to specialist doctors and services through your chosen medical practitioner. 

 

Aged Care

if you qualify we will request that your medical provider or nominated representative submit an ACAS referral via the My Aged Care website. 

You are required to pay for any other pharmacy or specialist services required. 

 

Service Environment

Electrical Appliances 

Residents who choose to bring in their own electrical appliances will be required to have them tested for safety by a licensed electrician on admission and annually. M&H Groupon will not be responsible for the cost of testing but service staff can arrange for testing for you at your request. 

Cleaning of rooms

Residents are encouraged to keep their rooms tidy and free of clutter for the safety of themselves and staff.  Facility staff will clean the rooms weekly, including vacuuming, dusting and wiping down hard surfaces.. You will be supplied with fresh bed linen weekly or as required. Rooms should be available for cleaning from 10 a.m. to 5 p.m.

Smoking

No smoking or use of electronic “E-Cigarettes” is allowed anywhere in the buildings or grounds outwit designated smoking areas. Residents are required to use ashtrays provided and to avoid smoking around staff members. To ensure the safety and wellbeing of service staff we cannot supervise residents when they are smoking or vaping. 

Maintaining Your Accommodation

You agree not to damage Your Accommodation and to keep Your Accommodation in the same quality and standard of repair as at the Occupancy Date at Your expense, fair wear and tear excepted. 

 

Alcohol

Consumption of alcohol is prohibited within the premises unless being served as part of a special celebration. Consumption of any type of prohibited drug is completely prohibited and will be grounds for eviction should it occur. 

Laundry

Personal laundry will be collected according to the laundry schedule from rooms and washed on the premises.  Labelling of personal laundry items is at the residents preference and expense. &H Groupon Pty Ltd takes no responsibility for lost or damaged personal clothing items. Changes to routine timings will be provided in writing to you 7 days in advance of any change.
